If you want to appeal a decision regarding your eligibility or for the amount of social assistance that you are receiving, you must use the Social Benefits Tribunal Appeal Form.
 
Appeal forms are available on this website by clicking here.  You can also obtain them at the Tribunal office or at any Ontario Works or Ontario Disability Support Program office or community legal clinics.
 
When you get your form, and need help to complete it, you can either telephone or visit the Tribunal's office where a staff member will assist you.
